SELF MANAGEMENT EDUCATION
East Health Trust PHO provides group self management education workshops for people with long term conditions and their families. These are informative, practical sessions to help people manage their own health more effectively.
Six specifically designed courses are held by trained facilitators at East Health Trust PHO premises.
Weight Self Management (LiveWell)
Facilitated by a registered dietitian with input from a registered nurse.
Suitable for anyone 18 years and over with a BMI of 28 or more.
Duration of programme: 1.5 hours each week for 6 weeks
Programme covers:
- Mindful eating
- Portion control
- Goal setting
- Physical activity
- Supermarket tour/label reading
Chronic Disease Self Management (Stanford)
This is an evidence based programme formally developed by Stanford University, USA.
Suitable for people living with any long term health condition.
Duration of programme: 2.5 hours each week for 6 weeks
Programme covers:
- Healthy eating
- Action planning
- Reading nutrition labels
- Coping tools
- Depression management
- Problem solving
Diabetes Self Management (Stanford)
This is an evidence based programme formally developed by Stanford University, USA.
Suitable for any person with pre-diabetes or is living with diabetes (Type 1 or 2).
Duration of programme: 2.5 hours each week for 6 weeks
Programme covers:
- Monitoring
- Healthy eating
- Depression management
- Problem solving
- Reading nutrition labels
- Stress management
Diabetes Self Management
Facilitated by a registered dietitian with input from a registered nurse and a registered pharmacist.
Suitable for any person with pre-diabetes or is living with diabetes (Type 1 or 2).
Duration of programme: 2.5 hours each week for 6 weeks
Programme covers:
- What is diabetes?
- Healthy eating
- Goal setting
- Physical activity
- Supermarket tour/label reading
- Understanding your medications
Pain Self Management (Stanford)
This is an evidence based programme formally developed by Stanford University, USA.
For people living with chronic pain lasting for more than 3 months.
Duration of programme: 2.5 hours each week for 6 weeks
Programme covers:
- Healthy eating
- Problem solving
- Moving easy programme
- Communication
- Strategies for managing pain, fatigue and frustration
Smoking Cessation supported by CM Smokefree Services
For anyone wanting support to quit smoking.
All smokefree advisors are able to ensure ongoing access to funded Nicotine Replacement Therapy (NRT) and advice about other non-funded NRT and prescription medication.
The range of support includes:
- Drop-in clinics (casual appointments at local bases)
- Group based support (hourly sessions over 7 weeks)
- Home visits (one-on-one support)
- Quit Bus (travelling to local community spots)
- Rewards based programmes for stopping smoking in pregnancy
- Intensive support for people using mental health services
- Inpatient support (while staying in Middlemore Hospital)
